רוב המשחק יתרחש בשנת 2025, ויציג אירועים, נשקים וסביבה עתידנית.[1][2] העלילה מתרחשת במלחמה עתידית הפורצת בעקבות פריצה למערכות של ארצות הברית אשר מפנה את כלי הנשק שלה כלפיה.[3]הסיפור מתחיל בשנת 2025 בנו של אלכס מייסון (גיבור Call of Duty: Black Ops) , דוויד מייסון, ילחם במספר אתרים הכוללים את מרכז ארצות הברית ואת הפלישה הסובייטית לאפגניסטן. האויב המרכזי בסיפור יהיה ראול מננדז אשר ינסה לפרוץ לתוך הנשקים העתידניים של צבא ארצות הברית ולגרום להם לתקוף את העולם. אך לא רק נשחק בעתיד, אנחנו גם נשחזר זיכרונות של WOODS, שכרגע זקן מאוד, המון שאלות היו האם וודס עדיין בחיים, התשובה היא כן, וודס שרד את הפיצוץ עם קראבצ'נקו, וגם קראבצ'נקו שרד . נשחזר זיכרונות של וודס כשנחזור 20-25 שנה אחורה, ונשחק עם מייסון, וודס, בכל מיני משימות המהוות המשך לבלאק אופס הראשון, (ווייטנאם, הריגת קראבצ'נקו) החלק הזה לא פחות טוב מהחלק העתידני ואפילו מאוד כיפי. בניגוד למשחקים הקודמים בסדרה, ניתנת לשחקן מספר בחירות גורליות בעלילה שיכולות לשנות את הסיפור , לכן יש חמישה סופים שונים, כל אחד מעניין מאוד ושונה מאוד מהאחר , לדוגמה

4: From game menu click "Custom Games" , setup your game - map - bots and "Start Match"
