עלילת המשחק מתרחשת כ-11 שנים לאחר המשחק הקודם (מתאים כרונולוגית לאריכי יציאת המשחקים) וממשיכה בנקודה שבו הסתיים המשחק הקודם- אליס משוחררת מהמוסד לחולי נפש בו אושפזה, ועוברת לגור בלונדון עם הפסיכיאטר שלה, לה הוא עוזר בעקבות הסיוטים החוזרים ונשנים הקשורים במותם הטראגי של הוריה בשריפה.
לאחר 11 שנים של שקט יחסי, דעתה של אליס מתחילה להתערער שוב, לאחר שהיא נחשפת לעובדה שמותם של הוריה לא היה "תאונה מצערת" כפי שחשדה עד כה.
